Case Manager, Community Support Program

Our Community Support Program is looking for an energetic, reliable, friendly and organized case manager to help community members learn to be more self-sufficient. This position requires knowledge of Fairfield County’s community supports and resources. Must possess effective communication skills, be able to work as a member of a team across different program within the agency, be able to learn multiple electronic health records, have basic knowledge of psychiatric diagnosis and experience working within the mental health field. Be able to spend 50% of the work week in the community and 50% in the office. Have a valid driver’s license/insurance and vehicle. This position is 35 hours per week, must be willing to work during nontraditional hours if needed.

Education: Bachelors degree or 10 years’ experience or Certified Recovery Support Specialist title.
