RYAN WHITE HIV/AIDS SERVICES

LifeBridge offers a comprehensive Medical Case Management system of care for people infected and affected by HIV disease, and delivers these services in a relevant manner, considering clients’ ethnicity, culture, level of understanding, and lifestyle. Persons are greeted in a warm and caring manner and confidentiality is always respected and assured. Client empowerment is realized through support, resource awareness, and advocacy.

Medical Case Management

Medical Case Management services are a range of client-centered services that link clients with health care, psychosocial, and other services provided by trained professionals. The coordination and follow up of medical treatments are a component of Medical Case Management. These services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care through ongoing assessment of the needs and personal support systems of the client and other key family members. This includes all types of case management, including face-to-face meetings, telephone calls, and any other forms of communication.

Our services also include:

  • Assistance with enrollment to Connecticut AIDS Drug Assistance Program
  • Health insurance premium and cost sharing assistance
  • Mental health services
  • Medical transportation services
  • Emergency financial assistance
  • Housing services
  • Linkage/referrals to other community resources included but not limited to: LifeBridge Community Closet, local food banks, oral health services, substance abuse treatment, outpatient/ambulatory services, early intervention services

Eligibility Requirements

Documentation from a licensed medical provider with identified agency/medical provider logo stating client’s HIV/AIDS status.

Client must meet federal poverty level that is set by Ryan White Grantees. Verification that the client meets the current eligibility requirement must be obtained at time of service. Acceptable proof of income includes, but not limited to unemployment or pay stubs, Supplemental Security Income, Social Security Disability Income, income tax return, DSS Budget Sheet, etc. Client must provide all household income. Clients with no income must sign the Program Zero Income Affidavit.
