Confidentiality and Release of Information

Per regulations, all personal information shared by clients is confidential. We do not share your name or any details of your treatment without your explicit consent, as indicated by signing a form allowing us to do so. Visitors are also required to follow and maintain the same confidentiality practices. The only exceptions to this are if there is a healthcare emergency, known child or elder abuse (we are mandated reporters), if you are at risk of harming yourself or others, or if a judge signs certain legal proceedings. If you choose to receive text messages from us, rest assured that your information will never be shared with third parties.

By opting into SMS from a web form or other medium, you agree to receive SMS messages from LifeBridge Community Services. This includes SMS messages for appointment scheduling, appointment reminders, post-visit instructions, lab notifications, and billing notifications. Message frequency varies.
